Information Sharing Authorization

By proceeding, you understand and agree that Sequoia Financial Advisors, LLC ("Sequoia"), partnership with Community Capital Technology Inc. (“Community Capital”), may request and share information you provide about you and/or your business as part of your financing inquiry with selected third-party financial institutions and/or other originators (“Lenders”) to help identify and facilitate financing opportunities.

The information shared may include, as relevant to your lending request:

• Name, business name, contact details, social security number, date of birth
• Financial statements, income tax returns and related documents
• Credit and asset information

• Other information you voluntarily provide to support the credit request

The parties that may have access to your information include:

• Lenders that you authorize us to engage for credit evaluation

Use and Protection.  All parties are expected to handle your information in accordance with applicable privacy and data protection laws and to use it solely for the purpose of evaluating and facilitating your financing options.
    
Ongoing Authorization. Your authorization applies to your current and future credit requests submitted through Sequoia Lending Solutions and remains in effect until you revoke it by written notice to lendingsolutions@sequoia-financial.com.
